For the Consequent Event code, in addition to where and what happened, an extra space is provided at the beginning of the code to indicate the type of event misadventure, no-harm event, or near miss (planned/unplanned recovery).

1= No recovery, harm to patient (Misadventure)
2= No recovery, no harm to patient (No-harm event)
3= near miss, unplanned recovery
4= near miss, planned recovery

The event in this scenario would be classified as a near miss or "4" because there was a planned recovery and the technologist who caught the error was following protocol.
