Another limitation in the intentional violation of safe practices standard can be seen in a circumstance common within most organizations. Short cuts in some procedures can be so widely practiced that they become the unwritten rule. Because these violations become incorporated into the normal work flow of an organization, individuals are not aware of the increased risk they are causing.

If an employee fears discipline, he or she may not report these behaviors and may, therefore, unknowingly increase risk to the patient.

It is the correction of these at-risk behaviors that provide the greatest opportunity to prevent future events.
