When determining which category should be associated with a particular root cause, there is a specific order in which to ask questions.

Asking first whether or not the root cause results from technical factors helps counteract the tendency to start and end with human factors, ignoring the latent conditions that set the human up to fail.

Looking for latent error first whether from technical or organizational factors increases the likelihood that all the causes underlying the event will be discovered.
