Frequency distribution of consequent (discovery) events
This bar graph of event code categories gives an overall view of where in the process of transfusion the high-risk events are being discovered.

For example, the bar for OE (Order Entry), tells us the number of times an event is discovered in the Order Entry process. An example of an event that could be discovered during Order Entry is a labeling problem on a tube of blood.
Remember because consequent events refer to where the event was discovered, learning about consequent events will not necessarily indicate where something went wrong.
