For each event with a high risk index, the MERS-TM process enables you to look in depth at why the event happened. However, discovering why is not as simple as it seems.
Remember the opening story and the misadventure at the red light? On the surface, the cause for the car accident was that the driver ran the red light. However since we can assume the driver knew to stop on red, we must look deeper at the underlying causes for the event to be fully understood and potentially prevented in the future.
