Risk equals the probability of a severe outcome to future patients multiplied by the probability
of recurrence (will the event happen again?).
Both the potential severity and the probability of recurrence for a specific event are categorized
into one of six levels very low, low, medium, high, very high, extremely high.

These assessments for severity and recurrence can then be multiplied together
to produce a risk index.
