Compare the situation involving intentional violation or intent with the following situation involving human error.

If, for example, the nurse inadvertently forgot to check the wristband, the label of human error would apply there was no intent to violate a safe rule or practice.

A system that acknowledges the unintentional nature of human error and seeks to learn from mistakes will encourage this nurse to report the error and participate in its investigation without fear of disciplinary action.
